The Evaluation of the Dibetes Mellitus Patient’s Referral System Between Chaiyaphum Hospital and Primary Care Units in Mueng District, Chaiyaphum Province

Abstract

The patient referral system between Chaiyaphum Hospital and Primary care unit was  connected between health facilities. Evaluation of system was conducted to analysis solution to the problems including the appropriated development. Objective : This research was evaluated the performance with CIPP Model and analysis solution to the problems including the development of diabetic patient’s referral systems between Chaiyaphum hospital and primary health care units in Mueng District, Chaiyaphum Province. Method : This was evaluation research used the CIPP Model such as context, input, process and output. Collected data in 75 authorities responsible diabetic patient’s referral systems, 300 patients with diabetes who were refer from Chaiyaphum hospital to primary health care units and 847 medication records of diabetic patients by using interviews and questionnaires.  Data were analyzed using content analysis of issues and statistics by frequency, percentage, average, minimum, maximum . Result : the objective of health care in patient with chronic disease was in the same direction both hospital and primary care unit which was to help the patients receiving healthcare service continuously, to prevent and reduce the incidence of complications. The evaluations of the input was the lack of healthcare providers. The evaluation of processes was found that the most problem of the process was drugs distribution. The evaluation of results was that both healthcare provider and patients were satisfied for referral system in high level . After implemented of referral system was founded patient who could control blood sugar level were 34.71 percent of all over refered patients while, 19.83 percent had kidney complications, 14.88 percent had eyes complication, and 3.54% had feet complications.The important problems were incorrect of medication lebel and data of drug used, patient’s information was not complete, lacking of healthcare providers and disconnected computer system respectively. For the suggestion were to establish a coordination center, computer system should be link together for the whole to system and also establish training program for gain the ability of diabetes care for healthcare providers and health volunteers. Conclusion : This reseach shown  that appropriated development patient’s referral systems were to establish a coordination center, computer system should be link together for the whole to system and also establish training program for gain the ability of diabetes care for healthcare providers and health volunteers.
