Abstract

บทคัดย่อ
ภาวะสมองขาดเลือดที่เกิดภายหลังการแตกของหลอดเลือดสมองโป่งพอง (Delayed cerebral ischemia หรือ DCI) เป็นปัจจัยสำคัญอันหนึ่งที่อาจส่งผลต่ออัตราการเสียชีวิต และผลการรักษาภายหลังการ ผ่าตัด วัตถุประสงค์ของการศึกษานี้เพื่อศึกษาปัจจัยที่สัมพันธ์กับภาวะดังกล่าว โดยศึกษาจากฐานข้อมูลผู้ป่วยที่ มีการเก็บไว้แบบไปข้างหน้า (Retrospective cohort study) ในผู้ป่วยที่ได้รับการวินิจฉัยว่าเป็นโรคหลอดเลือด สมองโป่งพองแตก และได้รับการผ่าตัดหนีบหลอดเลือดสมองโป่งพองจำนวนทั้งสิ้น 191 ราย กำหนดให้ผู้ป่วยที่ มีภาวะ DCI ทุกรายเป็นกลุ่มศึกษา โดยเปรียบเทียบกับกลุ่มผู้ป่วยที่ไม่มีภาวะ DCI เป็นกลุ่มควบคุม นำปัจจัยที่ สนใจวิเคราะห์หาความสัมพันธ์ทีละปัจจัย (univariate analysis) โดยปัจจัยที่มีความสัมพันธ์อย่างมีนัยสำคัญ ทางสถิติถูกนำมาวิเคราะห์ต่อโดยวิธี binary logistic regression analysis (multivariate analysis) จากการ ศึกษาพบผู้ป่วยที่มีภาวะ DCI จำนวนทั้งสิ้น 104 ราย และผู้ป่วยที่ไม่มีภาวะ DCI จำนวนทั้งสิ้น 87 ราย พบปัจจัยที่มีความสัมพันธ์กับการเกิดภาวะ DCI อย่างมีนัยสำคัญทางสถิติ ได้แก่ ผู้ป่วยที่มีระดับการประเมิน ความรู้สึกตัวในระยะแรกด้วย Hunt and Hess grading ได้ระดับ 3 หรือ 4 (adjusted OR 6.1; 95% CI 2.8-13.3) การประเมินลักษณะของเลือดที่ออกใต้เยื่อหุ้มสมองด้วย CT brain โดยใช้ Modified Fisher grading ได้ระดับ 3 หรือ 4 (adjusted OR 6.2; 95% CI 2.8-13.5) และการมีเลือดออกในเนื้อสมองร่วมด้วย (adjusted OR 4.6; 95% CI 1.5-14.3)

 

Abstract
Delayed cerebral ischemia (DCI) is a major complication of aneurysmal subarachnoid hemorrhage (SAH). A significant proportion of patients with this condition have poor outcome. The objective of this study aims to identify factors related to this condition. The cohort data were analyzed retrospectively. One hundred and ninety-one paptients with aneurysmal SAH who underwent aneurysm clipping were enrolled in this study. One hundred and four patients with DCI were studied and compared with 87 patients without DCI. Factors associated with DCI were analyzed by using binary logistic regression analysis. The results indicated that the
significant factors related to DCI included Hunt and Hess grading of grade 3 or 4 (adjusted OR 6.1; 95% CI 2.8-13.3), modified Fisher grading of grade 3 or 4 (adjusted OR 6.2; 95% CI 2.8-13.5), and additional intracerebral hemorrhage detected on cranial CT (adjusted OR 4.6; 95% CI 1.5-14.3).
